Individual Health Care Plans
Individual Health Care Plans for children with medical needs
If your child has a medical condition that requires specialist care and/or extra support in school, please speak to the School Administrator who, with the support of other health professionals will draw up an Individual Health Care Plan. This will help your child to remain fully included in events at school and where necessary, adaptations and additional support can be sought.
We aim to make your child's time at St. Bartholomew's as full filling as possible and it may be that the completion of a plan will help to do this.

For Parents of children under the age of 5
We have close links with Health Visitors who with the school nursing team are based at Kidderminster Health Centre. Similarly this team are available to support you. If you think you may like to make contact with the Health Visitor but are not sure where to start, please speak to a member of the Nursery team or the School Administrator.
Please be aware that you can make contact with the Health visitors yourself on 01905 520032
